Medical House Calls | Inova Provide safe and effective primary care in collaborating with the interdisciplinary Medical House Calls team.
Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for the Commonwealth of Virginia and delineated in the written Scope of Practice/Protocol for Advanced Practice Professionals (Licensed Independent Practitioners).

Management of various Care Plans Performs new consultations, patient follow ups, daily rounding and procedure triaging.

Ensure key quality metrics are optimized.
Participates in quality improvement projects as assigned.

Minimum Qualifications
Education: Masters in Advanced Clinical Practice or graduate of a post masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing. For physician Assistants, graduate of an accredited Physician Assistant program and certification by National Commission on Certification of Physician Assistants.
