• Summary: The Psychiatric Mental Health Nurse Practitioner (PMHNP) will play a pivotal role in assessing, diagnosing, and treating residents in the long-term care facility setting. They will be responsible for developing personalized treatment plans for individuals with various behavioral health conditions including mood disorders, thought disorders, and substance use disorders. They will serve as leaders in a multidisciplinary team working to enhance the overall well-being of our residents.

• Responsibilities: Conduct thorough behavioral health assessments and evaluations for residents. Develop and oversee individualized treatment plans in collaboration with residents, families, and interdisciplinary teams Prescribe and manage appropriate medications and therapies tailored to the long-term care setting Provide counseling and support to residents and their families Monitor resident progress and adjust treatment plans as necessary Educate residents and staff about behavioral health conditions and available resources within the long-term care setting

• Education: Master’s or Doctorate Degree in Nursing with a specialization in Psychiatric nursing Graduation from a state-certified advance practice program specializing in psychiatry/mental health

• Licenses/Certifications: Must have a certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP) Must have an unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Ohio Must have a valid DEA number and unrestricted DEA license in Ohio Must have a state-issued Controlled Substance Registration (CSR) in Ohio Must be registered with the state Prescription Monitoring Program (PMP) in Ohio where direct services will be provided No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid, SCHIP) Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of
