Discharge Planner

The Social Worker provides comprehensive case management, discharge planning, continuing care services, and clinical social work, including crisis intervention, under department leadership. Responsibilities also include educating patients, families, and significant others, as well as providing community education as needed.

Candidates must possess a Master's Degree in Social Work or MHC from an accredited school and must be a NYS LMSW or LMHC at hire, or secure licensure within the probationary period. Prior psychiatric experience in a hospital or community health agency is preferred, and maintaining New York State third-party reimbursement authorization is desirable for LCSWs.
