The Nurse Practitioner or Physician Assistant is responsible for delivering high-quality primary healthcare services to geriatric patients, which includes diagnosing and treating both acute and chronic conditions. This involves collaborating with an interdisciplinary team to coordinate overall patient care, focusing on disease prevention and management across clinic, assisted living, and home settings.
Candidates must possess a Maryland NP or PA license, DEA registration, and appropriate board certification (e.g., PA-C, AGNP, FNP). While 1+ year of advanced practice experience is strongly preferred, up to 5+ years of relevant Registered Nurse experience may be substituted.
Nurse Practitioner or Physician Assistant - Geriatric Primary Care
This private practice opened in 2013 and currently employs an interdisciplinary team of physicians, nurse practitioners, physician assistants, behavioral health professionals, and nutrition professionals, along with a robust support staff. The practice provides a full scope of internal medicine and geriatric primary care services in a variety of settings and locations.
Providers primarily see patients in assisted living facilities (ALFs) and rarely in private homes (house calls).
The practice operates multiple clinics in Montgomery County and the new provider may have the opportunity for some telemedicine and/or in-person clinical days in an office.
Providers see 8 to 15 patients per day depending on setting, travel time, and types of appointments scheduled.
The practice uses the Office Ally EHR.
