The RN Acute Care Navigator oversees patient care, ensures seamless progression, coordinates services, and determines the appropriate level of care for their caseload, focusing on optimizing length of stay and preventing readmissions. This role involves collaborating with teams to conduct initial assessments, develop discharge plans, provide patient education on treatments, and monitor progress by reviewing health records.
Candidates must possess either a Bachelor of Science in Nursing (BSN) or five years of applicable nursing experience, along with a current, active, and unrestricted Registered Nurse license in the state of practice. Preferred qualifications include an MSN or higher, experience in patient advocacy, care management, and knowledge of hospital and community resources.
