The role involves conducting virtual outreach to patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs to review care plans and provide education. Key duties include monitoring patient health status via RPM tools, escalating clinical concerns, documenting interactions, and coordinating follow-up care.
Candidates must possess an active Nevada LPN or LVN license and have a minimum of two years of clinical nursing experience, preferably in telehealth or chronic care. Strong English communication skills and competence in patient documentation and clinical judgment are required.
PharmD Live seeks a proactive Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our remote care team. This position supports patients enrolled in chronic care management (CCM) and remote patient monitoring (RPM) programs. The ideal candidate will provide high-quality patient care, education, and coordination, ensuring adherence to care plans while collaborating with multidisciplinary teams.
Conduct virtual outreach with patients to review care plans and provide education Monitor patient health status via RPM tools and escalate clinical concerns appropriately Document all interactions in compliance with CMS, payer, and internal quality standards Coordinate post-discharge follow-up and specialist appointments Collaborate with nurses, pharmacists, and other care team members to optimize outcomes Foster patient engagement, adherence, and trust through empathetic communication
Active Nevada LPN or LVN license in good standing Minimum of 2 years of clinical nursing experience (telehealth or chronic care preferred) Strong verbal and written communication skills in English Competent in patient documentation and clinical judgment Experience with CCM and/or RPM workflows is a plus Ability to work independently in a HIPAA-compliant, remote environment
