The Patient Care Coordinator is the initial point of contact, responsible for complex, interdisciplinary patient care coordination involving both administrative tasks like scheduling and data entry, and clinical support such as recording vital signs under RN supervision. This role requires maintaining excellent customer service standards while ensuring patient confidentiality and compliance with regulatory requirements.
A minimum of a High School completion/GED is required, along with current HealthCare Provider or Professional Rescuer BLS CPR certification. Candidates must successfully complete one of several pathways demonstrating fundamental patient care skills, such as prior training (CNA/STNA) or current enrollment in nursing school with a completed clinical rotation.
