The Case Manager will manage client cases utilizing clinical expertise, providing case management and care coordination services within the facility. This includes developing and evaluating care plans, coordinating the continuum of care, and conducting psychosocial assessments and therapy sessions.
A Bachelor's degree in Social Work, Counseling, or a related human service field is required, along with at least one year of experience in a behavioral health setting. Candidates must also possess or obtain required certifications such as First Aid, CPR, and de-escalation/restraint training.
Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents.
• Perform other functions and tasks as assigned.
• Bachelor’s degree in Social Work, Counseling or related human service field required. One or more years of experience working in behavioral health setting. Experience with the patient population served by the facility preferred. Previous experience in case management and/or utilization review preferred.
Current license required within the state the facility is operating in if required. First Aid, CPR, de-escalation and restraint certification required (training available upon hire and offered by facility).
