Requirements summary
Candidates must possess a Bachelor's or Master's degree in Dietetics, along with current Registered Dietitian (RD) certification and Minnesota Licensed Dietitian (LD) status. Preference is given to experience in diabetes or pediatric care, and working with the Native American community.
This position is responsible for assisting Native American Community Clinic (NACC) Community Health Department preventative care and chronic disease management grant programming and quality improvement projects through patient outreach and education. The Registered Dietitian will be responsible for assessing nutritional status and planning the appropriate nutritional care for patients in the outpatient and community setting.
Essential duties and responsibilities
Assess patients’ nutritional status and needs, evaluate and chart results of nutrition counseling using Nutrition Care Process
Provide individual and group nutrition counseling including goal setting
Actively participate in integrated care team with medical providers, community pharmacists and other healthcare professionals in order to coordinate medical and nutritional needs
Manage chronic condition population patient panels through registry monitoring and patient outreach following clinical protocols.
Maintain timely follow-up care with all nutrition referrals and appointments
Act as a nutrition information resource for medical providers, nursing and other clinic personnel, and the community
Deliver community nutrition programs
Develop, acquire and maintain culturally appropriate nutrition education materials
Phone and mail outreach for population health management. Includes scheduling appointments and basic care coordination.
Recruit for chronic disease management groups and preventative care events and programming.
Support grant specific program evaluation activities and incorporating findings to refine and improve program effectiveness to meet grant deliverables.
Work with grant/project evaluators and Community Health Manager to develop data and reports as required to fulfill requirements of program evaluation plans.
Maintain regular communication with project partners and with the project officer to fulfill grant requirements, including submitting regular reports, as required.
Lead employee wellness initiatives via the Health Equity Committee with support from Community Health Manager.
Support ongoing sustainability planning on behalf of Community Health Programs.
Play an active role on the outreach team. Other duties may be assigned.

Education and/Or experience
Bachelor or Master's degree in Dietetics
Registered Dietitian (RD) from Commission of Dietetic Registration required
Licensed Dietician (LD) from MN Board of Dietetics and Nutrition Practice required
Preference to experience providing diabetes or pediatric care
Preference to experience working with the Native American community
Language skills

Mathematical skills
