The Hospice Aide will provide direct patient care to patients under the direction of the RN, following the established Hospice Aide/Homemaker Plan of Care. This includes assisting patients with safe transfers and ambulation, reporting changes, and documenting all care provided to ensure continuity.
Candidates must possess an unencumbered certification according to the applicable Nurse Aide Registry Licensure, meaning a current CNA license is mandatory. A high school diploma or GED is preferred, along with at least one year of experience as a Hospice Aide or Nursing Assistant in a relevant facility, plus reliable transportation and valid auto insurance.

Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care. Practice accepted infection control principles.
