The Licensed Practical Nurse will conduct medical assessments, support individuals in medical appointments, and collaborate on treatment plans. They will also provide education on self-care and medication management, and assist in crisis situations.
Candidates must be a graduate of a nursing school with LPN licensure and have experience with patients with complex medical and behavioral health issues. CPR & First Aid Certification is required or must be obtained within 90 days of employment.

About the program: Hendee Road House
Hendee Road Home is a 6 bed residential program for men with severe mental illness and are discharging from long term hospitalization. The program will address the full range of daily living needs of each individual with a focus on increasing independence and self-efficacy of individuals through 24-hour/7-day a week staff support in the residence and the community. Staff interventions focus on teaching skills around daily living, symptom management, coping skills, medication management, health & nutrition, healthy relationships, personal safety, money management, substance use education, linking to recovery supports, and providing sober activities. Staff will also provide crisis intervention in attempt to reduce need for hospitalizations and emergency room visits.
About the Position: Licensed Practical Nurse
Graduate of school of Nursing with LPN licensure.
Experience with patients with complex medical and behavioral health issues.
Licensed Practical Nurse in State of CT. Valid driver’s license.
Demonstrated education, supervised training, and practical experience providing the full spectrum of behavioral health treatment, including clinical. CPR & First Aid Certification required OR must obtain within 90 days of employment.

$22.72/hour minimum, and up commensurate upon experience
*Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
