The Care Coordinator facilitates the Wraparound model to develop individualized care plans for children and youth with complex behavioral needs. They provide ongoing advocacy, coordinate team meetings, and link families to essential community resources and supports.
A Bachelor's degree in a Human Services related field and at least 2 years of experience in a mental health setting are required. Candidates must possess a valid driver's license and obtain CPR/First-Aid certification within six months of hire.

About the program: Care Coordination
Care Coordination at CHR is an intensive program for children and youth ages 0-18 (or up to age 22 if in school) with emotional/behavioral difficulties and complex service needs and their families. Care Coordinators assist the family through a child-specific team that develops individual and family-focused service plans. The coordinators also provide ongoing consultation with the child-specific team to provide linkage, coordination, follow-up, and advocacy for the child and family
Bachelor’s Degree in Human Services related field required.
Minimum of 2 years' experience in mental health setting.
Valid driver’s license. CPR/First-Aid Certification within 6 months of hire.

*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
