The Social Worker assesses and plans interventions to help patients cope with various problems. They also participate in case reviews and consult with healthcare team members to ensure compliance with treatment plans.
A Bachelor's degree is required, with a Master's preferred. Candidates must obtain relevant social work licenses within six months of hire and complete specific certifications within designated timeframes.

How you will contribute The Social Worker (LSW) is responsible for Assessing, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, priorities, provides and/or arranges for needed internal and external services/interventions. Participate in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Supervises and/or trains new staff, students, and interns. Provides individual, family, and group therapy. Non-Essential Functions. What we are looking for Education: Bachelors required; master is preferred Licenses: Board Certified Social Worker, or related field (i.e., counseling) in the state of Pennsylvania (CSW, LPCA, LCSW) within 6 months of hire Certifications: Handle with Care within 30 days of hire. Required Skills Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. You must be able to work in a stressful environment and take appropriate.
