The Nurse Coordinator is responsible for coordinating the health care of select patient populations across the continuum of care. This includes assessing patient needs, facilitating education, and ensuring safe and appropriate patient care.
A nursing diploma or associate degree is required, with 2-5 years of experience preferred. A bachelor's degree in nursing is preferred, along with oncology experience.
This Nurse Coordinator role will work with our hematologic malignancy program supporting one lymphoma provider.
Monday - Friday 8 am - 5 pm No Weekends, Holidays or Call Potential work from home option one day per week after a year Full time, Salaried & Benefits eligible Additional Preferred Requirements Oncology experience strongly preferred BSN preferred
Role Purpose Coordinates the health care of select patient populations/designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient/family for procedure/test in collaboration with the healthcare team.
