The APP will conduct new and follow-up patient visits, manage liver-related conditions, and coordinate care for high-acuity patients. They will also collaborate with the care team and contribute to quality improvement initiatives.
Candidates must be licensed Nurse Practitioners in Iowa with a minimum of 2 years of experience in GI/hepatology or a related specialty. Strong communication and critical-thinking skills are essential for managing complex patients.
Monday-Friday 8-4:30 Conduct new and follow-up patient visits, including assessments, diagnosis, treatment planning, and ongoing management of liver-related conditions. Coordinate care for high-acuity and chronically ill patients, ensuring seamless communication across the care team. Order and interpret diagnostic tests, labs, imaging, and specialty evaluations. Manage medication adjustments, including hepatology-specific therapies and monitoring for side effects. Provide patient and family education related to liver disease, treatment plans, lifestyle interventions, and long-term care needs. Participate in care pathways for cirrhosis management, transplant evaluations, and post-transplant follow-up as applicable. Collaborate with inpatient teams to support transitions of care and reduce readmissions. Document all patient interactions in the electronic medical record accurately and in a timely manner. Contribute to quality improvement initiatives, care standardization, and process development within the clinic. Outreach clinic at least once monthly and could involve more pending volumes.
Licensed Nurse Practitioner (NP) in the state of Iowa Minimum of 2 years of experience in GI/hepatology, internal medicine, or a related specialty preferred. Demonstrated ability to manage complex patients in an outpatient specialty setting. Strong communication, critical-thinking, and patient education skills. Ability to work effectively in a multidisciplinary team. EMR proficiency (Epic preferred).
