As a Provider at Lucet, you will deliver comprehensive care to patients in their homes and community-based services. This includes conducting in-home visits, managing chronic conditions, and collaborating with interdisciplinary teams to ensure coordinated care.
Candidates must have active, unrestricted licensure as a Provider and certification as a Family Nurse Practitioner or Physician Assistant. Preferred qualifications include two years of experience in a primary care setting and the ability to pass a comprehensive background check.

As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
