Advance Practice Provider (Nurse Practitioner or Physician Assistant) - Full-Time Clinical Team Care Model Role - Internal Medicine

Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant to join our Internal Medicine team in a unique Full-Time role. In this role, you will spend approximately 50% of your time providing direct patient care and 50% supporting the provider team through clinical coordination and workflow support. About the Role

As an Advanced Practice Provider at FHMG, you will function as a clinician, consultant, educator, and collaborative partner to physician colleagues. You will help coordinate high-quality, patient-centered care while supporting practice efficiency, population health initiatives, and quality improvement efforts.

You will provide comprehensive evaluation and management services to adult and geriatric patients, grounded in evidence-based medicine and aligned with national quality standards such as Patient-Centered Medical Home (PCMH) principles.

• Key Responsibilities
• Provide high-quality, evidence-based care to adult and geriatric patients, including history taking, physical exams, diagnosis, treatment planning, and patient education
• Spend approximately 50% of time in direct patient care and 50% supporting physician colleagues and clinical workflows (no independent patient panel)
• Order and interpret appropriate labs, imaging, and referrals; perform procedures within scope of practice
