High School/GED preferred and Community Health Worker certification REQUIRED.

Special skills

Medical knowledge and ability to work with an Electronic Medical Record.

Primary functions

• Provide health education and information with clients.
• Coordinate care between individuals, communities, and health and social service systems.
• Be a liaison between providers and clients.

Shift/Days and Work Hours

• Monday –
• Friday, 8:00am – 4:30pm
