Requires an active ANCC or AANP certification in Family, Adult, or Geriatric Primary Care and an unrestricted Missouri license. Must possess a valid driver's license and reliable transportation for daily travel to patient homes.
$20,000 Student Loan Repayment or $10,000 Sign-on Bonus
In Home Care, No Nights, Weekends or Holidays This position requires daily travel to patient homes in Buchanan and Andrew counties.
The Optum at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of patients (beneficiaries) in their place of residence. The APC within OAH provides care to our highest-risk health plan beneficiaries and is part of an interdisciplinary team. Conduct comprehensive in-home assessments, including: Past medical history review, medication reconciliation, vital signs, and physical exam Evidence-based screenings and point-of-care testing (as appropriate) Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions Manage members during a short term sub-acute rehabilitation episode Ensure accurate and complete documentation, including ICD 10 conditions Communicate and collaborate with the interdisciplinary care team Conduct advanced illness and advanced care planning conversations Provide patients and caregivers with counseling and education

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Active, unrestricted licensure in Missouri (or ability to obtain by start date) without current disciplinary actions or practice limitations Active, unrestricted driver’s license and access to reliable transportation Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties
1+ years of clinical experience as an APC (family, geriatric, or home health) Experience in geriatric medicine, long-term care, senior living or home care setting Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficiency with electronic medical records and technology
