The Case Management Assistant supports the treatment team by interviewing patients to document baseline functioning and facilitating post-hospital care transitions. They ensure accurate communication and provide an excellent patient experience throughout the hospital stay.
Candidates must hold an Associate's degree with one year of healthcare experience or a Bachelor's degree in social work or a related healthcare field. Proficiency in computer software and strong interpersonal and analytical skills are required.
The Case Management (CM) Assistant works collaboratively with Case Management and the Treatment Team to support timely patient progression to the appropriate level of post-hospital care throughout the patient’s hospital stay. Early in the patient’s hospital stay, the CM Assistant will interview the patient, caregiver, and/or decision-maker to document the patient’s baseline functioning in the pre-hospital environment and complete the discharge evaluation as required by Centers for Medicare & Medicaid Services (CMS). Through the patient’s hospital stay, the CM Assistant interacts directly with patients, caregivers, and/or decision-makers and gathers pertinent patient information for continued care transition needs while providing excellent patient experience. Ensures all communication is appropriate, accurate, timely, and consistent with the patient’s plan of care.
Education: Bachelor’s degree in social work (BSW) Experience: Social work internship in a healthcare setting Home care, post-acute care, discharge planning and care coordination, and/or pertinent experience. Other Skills/ Knowledge: Working knowledge of computer software applications and electronic health records. Familiarity with medical terminology.
