The Intensive Family Support Coordinator conducts home visits to assess family needs and develops personalized support plans to foster independence. They also assist families in navigating state and federal entitlement benefits and coordinate care with clinical and medical providers.
Candidates must hold a Master’s degree in a human services-related field and possess at least two years of direct family support experience. A valid driver's license and the ability to pass background and reference checks are also required.

The role!

Tiered pay increases based on years of service, pending performance evaluations 4 weeks’ vacation, 1-week sick time, and 12 holidays for new full-time employees Low-cost benefit plans – Medical & dental insurance, employer-paid life insurance, and long-term disability coverage Tuition reimbursement eligibility after 90 days of employment Summary & Responsibilities Conducts home visits to assess family needs and identifies ways to access community supports, benefits, and DDS supports based on those needs. Facilitates the development of a Family Support Plan that incorporates the individual’s needs and supports their growth and independence. Supports families in accessing services and supports available within the community. Practices good judgement in responding to families navigating mental health or behavioral challenges by providing trauma informed care. Assists families in obtaining state and federal entitlement benefits, including SSI, SNAP, MassHealth, subsidized housing, and transportation benefits. Assists families in accessing clinical and medical supports, including behavioral supports, therapy, counseling, nursing, and related services. Collaborates with clinical and medical providers to support coordinated care for individuals and families. Facilitates the use of DDS flexible funds to assist families with short-term needs that cannot be met through existing services and supports. Works with local hospitals, schools, and community service organizations to identify potential support personnel, including respite providers, PCAs, and skills instructors.
Master’s degree in a human services–related field (including but not limited to social work, education, psychology, public administration, or public health) A minimum of two years of experience providing direct support to families.
