The Care Coordinator provides education, support, and resource linkage to individuals managing mental illness or substance use disorders. They also assist clients in developing independent living skills and maintain accurate clinical documentation.
Candidates must hold a bachelor's or master's degree in a human services field, or possess specific professional certifications or equivalent experience. Qualifying experience in mental health or substance use services is required.
The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and manage illness to be successful in the living, working, learning and social environments of their choice. CC’s teach, model and practice skills with person(s)-served to promote recovery and wellness.
