Must be a licensed Physical Therapist Assistant (PTA) in the state of New Mexico. Requires a valid driver's license, reliable transportation, and the ability to work independently in patient homes.

Key Responsibilities Provide in-home physical therapy treatments as delegated by a licensed Physical Therapist Support individualized care plans and track patient progress toward functional goals Educate patients and caregivers on exercises, mobility, and home safety Communicate observations clearly with supervising PT and interdisciplinary team Document all visits and care provided in accordance with agency and regulatory standards Travel to patient homes throughout the service area (mileage reimbursed) Participate in team meetings, care planning, and ongoing education Uphold agency standards for compassionate, respectful, and compliant care
