Provide physical therapy services to adult and geriatric patients while collaborating with a clinical team. Maintain accurate documentation, including progress notes, discharge summaries, and individualized plans of care.
Candidates must hold a valid State Physical Therapy Assistant License and Credentials. Previous experience in skilled nursing facilities or acute care settings is required.
