The Health Coach promotes disease management and preventative care by engaging patients in healthy lifestyle choices. They collaborate with the Complex Care Management team to monitor patient progress and ensure adherence to comprehensive care plans.
Candidates must have an associate degree and at least two years of experience in a clinical, wellness, or health coaching setting. Proficiency in managing chronic illnesses and strong interpersonal skills are required for effective patient communication.
The Health Coach serves as a key member of the Complex Care Management team and Primary Care team to promote disease management, preventative care, and wellness. Promotes, encourages, and engages the patient to take an active role in their health by providing them with tools necessary to make healthy life style choices and adopt life-long healthy behaviors. In collaboration with the Complex Care Manager, coordinates, monitors, and follows up with patient to ensure patient they are working toward the comprehensive plan of care and care goals. Performs ongoing communication and works to promote quality of care through collaboration with the Complex Care Manager and all service team members, patients and families. Works with the Care Management Division leadership team to ensure alignment with the strategic goals to meet OSF Key Results.
Experience: Adult and/or pediatric care management experience Licensure/Certifications: Current state licensure as LPN Other requirements/information: Broad knowledge of chronic illness and medical and community resources Positive, empathetic and interpersonal skills to communicate effectively to patients and families with a variety of health issues, age groups and socioeconomic backgrounds Demonstrated ability to successfully use independent judgment and make decisions, with little supervision Ability to work with others and efficiently with excellent time management skills OSF HealthCare is an Equal Opportunity Employer.
