The Physical Therapist plans, coordinates, and implements home health and hospice therapy services to meet the needs of patients and their families. They are responsible for conducting comprehensive assessments, developing individualized care plans, and collaborating with the healthcare team to ensure quality outcomes.
Candidates must hold a degree from an accredited physical therapy program and possess a valid state license to practice. A minimum of two years of experience is required, with preference given to those with a background in community or home health settings.
$39-$60.52 per hour, based on experience.
Full-time 32-40 hours per week.
This role involves local/regional travel throughout Central Oregon via personal vehicle during the workday, including visits to patients' homes and facilities.
The Physical Therapist plans, coordinates, implements and evaluates home health and hospice therapy services. The Physical Therapist is knowledgeable in providing therapy in the home setting. The therapist builds from the resources of the organization and community to provide services to meet the needs of individuals and families within their homes. The Physical Therapist is responsible for cost effective and effective patient care with quality outcomes.
