Specialist, you will provide:
Assessments and
Intake: Conduct Comprehensive Needs Assessments (CNA) with clients and guardians (if applicable). Complete intake forms, including obtaining appropriate releases of information. Write and document CNA narratives in the Electronic Health Record (EHR). Populate client profiles in the EHR, including diagnoses, medications, allergies, and other critical information.
Service
Coordination: Collaborate with associated professionals involved in the client’s care. Recommend appropriate services based on the CNA findings. Complete the referral management workflow and service authorization process. Submit Service Authorization Forms via appropriate channels (e.g., MCO portals).
Communication and
Support: Respond promptly to all emails and voicemails. Interact with MCO Care Coordinators for service justifications and additional information requests. Notify leadership and authorization specialists upon completion of assessments.
Administrative
Tasks: Participate in team meetings and case staffing with supervisors. Maintain office hours to assist clinical teams with completing assessments and authorizations. Ensure timely uploading and addressing of relevant documents in the EHR. Why Join NCG?
