The Social Worker provides clinical support to hospice patients and their families in home and facility settings. They assess cultural, spiritual, and emotional needs to offer comprehensive end-of-life care services.
Candidates must hold a Bachelor's degree in Social Work and a valid state license (LSW or LISW). A valid driver's license, reliable transportation, and one year of healthcare social work experience are required.
We provide superior care for patients in families in homes/facilities. The Medial Social Worker will utilize their advanced educational background, their clinical expertise, judgment and discretion, to help identify the patient and family's unique cultural/spiritual beliefs, coping skills, pre-bereavement and anticipatory grief needs to offer a wide range of services to terminally-ill patients and families.
