The Physical Therapist performs patient evaluations, develops and implements treatment plans, and provides therapeutic interventions. They also participate in interdisciplinary discharge planning and ensure high-quality, safe patient care in accordance with regulatory standards.
Candidates must hold a current Illinois Physical Therapy license and be a graduate of an accredited Physical Therapy program. Demonstrated clinical competencies, CPR certification, and the ability to perform physical tasks like patient transfers are required.
Performs Physical Therapy (PT) evaluation, reassessment and consultation; provides physical therapy plan of care, goals, discharge planning, patient and significant other (SO) education; renders physical therapy services, interventions, use of therapeutic modalities within the practice guidelines of the American Physical Therapy Association (APTA) and other regulatory agencies; and participates in collaborative patient care services.
