The Physician provides comprehensive primary care in an outpatient setting, focusing on chronic disease management and preventive health. Responsibilities include developing patient care plans, coordinating specialist referrals, and supervising advanced practice providers.
Candidates must hold an MD or DO degree, complete an accredited residency, and be Board Certified or Eligible. Active state medical licensure, DEA certification, and CPR certification are required.
Physician – Primary Care / Outpatient SettingPosition Overview We are seeking a dedicated Physician to provide comprehensive, patient-centered care in an outpatient clinical setting. This role focuses on managing chronic conditions, promoting preventive care, and coordinating treatment plans to ensure continuity and quality outcomes. The Physician will collaborate closely with clinical staff and contribute to a team-based care environment. Key Responsibilities Develop and implement effective patient care plans, including diagnostics, treatments, referrals, and follow-up care Manage continuity of care, including scheduling, tracking return visits, and monitoring chronic conditions and abnormal results Provide whole-person care by addressing medical, social, psychological, and environmental factors Coordinate referrals to specialists such as social workers, nutritionists, and mental health professionals Maintain accurate, concise, and timely medical documentation Perform and document routine health screenings and preventive care services Educate patients on diagnoses, treatment plans, medications, and preventive health strategies Follow infection control protocols and universal precautions Supervise and collaborate with advanced practice providers when applicable Provide consultation and oversight for patient care as needed Participate in quality assurance and performance improvement initiatives Attend provider meetings, trainings, and required in-service sessions Contribute to clinical guidelines and protocol development Engage in community outreach and education when requested Support teaching and mentoring of students or residents as applicable Perform additional duties as assigned Education & Training Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution Completion of an accredited residency program Board Certified or Board Eligible Active or eligible for state medical licensure DEA certification Current CPR certification Commitment to continuing medical education Experience 1–3 years of clinical experience preferred, but not required Skills & Competencies Strong patient-centered care and customer service skills Excellent organizational and time management abilities Ability to work effectively within a collaborative team environment Strong written and verbal communication skills Adaptability and flexibility in a dynamic clinical setting Technical Skills Familiarity with electronic health records (EHR) systems Basic proficiency with Microsoft Office and standard office equipment Additional Requirements Valid driver’s license and reliable transportation Participation in required training and quality improvement initiatives
