The Physical Therapist will deliver physical therapy services according to the physician's plan of care, assess patients' therapeutic status, and adjust treatment plans as necessary. They will also provide education to patients and caregivers and contribute to discharge planning.
Candidates must have an active California Physical Therapist license and a valid driver's license with auto insurance. A minimum of one year of relevant experience and current CPR certification are also required.

Deliver physical therapy services in accordance with the physician’s written plan of care, including but not limited to: Assessing and evaluating patients’ therapeutic, rehabilitative, and functional status. Participating in the development and adjustment of the overall plan of care. Assessing muscle strength, mobility, gait, range of motion (ROM), and rehabilitation potential. Administering and directing physical therapy treatments. Instructing patients and caregivers in the use and maintenance of therapeutic equipment or appliances. Identifying and prioritizing physical therapy needs. Reporting patient responses to treatment or changes in condition to the physician. Initiate physical therapy programs and guide family members, caregivers, or other personnel in specific phases of therapy to support patient progress. Provide education to patients and caregivers regarding therapy goals and techniques, participating in case conferences as necessary. Identify patient and family/caregiver needs for additional home health services and make appropriate referrals. Prepare and submit clinical documentation and progress reports based on patients’ goal attainment. Contribute to discharge planning to ensure a smooth transition of care.
Maintain accurate and timely clinical notes and provide updates to the primary physician as needed. Communicate effectively with physicians regarding patients' conditions, needs, and any necessary adjustments to care plans, obtaining orders as required.
Qualifications and Skills: Active and unencumbered California Physical Therapist (PT) license. Valid California driver’s license and proof of auto insurance. Minimum of one (1+) year of relevant experience. Current CPR certification.
