Position: Home Health Aide (HHA/CNA)

Location

Based in Orange Park, FL.

What You’ll Do

As a Home Health Aide (HHA/CNA), you’ll be a vital part of the care team—supporting patients with personal care needs while promoting dignity and independence.

Key responsibilities include

Assisting with personal care, bathing, and hands-on care in accordance with the patient's plan of care
Accurately and promptly documenting each visit per agency policy
Notifying the supervising RN of any changes in the patient’s condition

**Note

This role involves multiple short visits throughout the day—not shift work or extended 12-hour cases.

Qualifications

Completion of a Home Health Aide training program and competency evaluation meeting Medicare’s Conditions of Participation
Valid driver’s license, reliable transportation, and auto insurance (required)
Ability to travel to patients’ homes throughout the day
Previous home health experience is a plus, but not required
