The Case Manager II provides comprehensive case management services to program participants, focusing on securing stable housing and supporting recovery from substance abuse and mental health conditions. Responsibilities include developing individualized care plans, facilitating life skills groups, and coordinating with multidisciplinary teams to ensure effective service delivery.
Candidates must possess at least a bachelor's degree in behavioral health or a related field and one year of experience in case management. Experience working with the Alaska Native/American Indian population and knowledge of substance abuse and mental health disorders are required.
The goal of the New Beginnings Recovery Home Project is to provide comprehensive, coordinated, and evidence-based treatment and services for) individuals experiencing substance abuse and co-occurring mental health conditions, who are experiencing homelessness resulting in reduce substance abuse and improve housing stability.
This position is responsible for providing case management services to program participants (consumers). The role works in coordination with the BHS multidisciplinary team to support consumers in securing and maintaining stable housing. Within the New Beginnings program, this position emphasizes strong collaboration, flexibility, and a team-oriented approach to effectively meet both programmatic requirements and individualized consumer needs.
