The COE Case Manager assesses new referrals, coordinates services with medical and counseling teams, and monitors patient progress through treatment. They also facilitate interdisciplinary meetings, assist with discharge planning, and provide ongoing support to residents.
Candidates must possess a Bachelor’s degree in a Human Service field from an accredited institution. A valid driver's license is required, and the role may involve up to 15% travel.
