Provide safe and direct personal care to patients in their homes under professional supervision to achieve identified health goals. Act as an extension of skilled nursing and rehabilitation services while maintaining accurate documentation and reporting patient changes.
Requires a High School Diploma or GED and a valid Certified Nursing Assistant (CNA) license. Must possess a driver's license, automotive insurance, and CPR certification based on AHA standards.
Attends compliance training and adheres to the organization standards of conduct, policies and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional. Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization. Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Proven ability to facilitate change. Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. Participates in all required agency meetings. Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff. Participates in all required agency meetings. High School Diploma or GED required One (1) year experience preferred
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire Automotive Insurance Certified Nursing Assistant (CNA) Driver's License
