The Physical Therapist is responsible for evaluating patients, developing treatment plans, and providing direct rehabilitative care to improve mobility and function. They also collaborate with interdisciplinary teams, supervise assistants, and provide training to facility staff.
Candidates must hold a B.S., Master's, or DPT degree in Physical Therapy and possess a valid state license. Strong interpersonal skills and the ability to maintain professional documentation are essential for this role.

As a Physical Therapist at Concept Rehab, you will provide evidence-based evaluation and treatment to patients recovering from illness, injury, or surgery in a post-acute rehabilitation setting. Key Responsibilities Evaluate patients within 24–48 hours of physician orders Develop and implement individualized treatment plans Provide skilled, patient-centered therapy interventions Supervise and collaborate with Physical Therapy Assistants Communicate effectively with physicians, nursing, families, and care teams Participate in interdisciplinary and care planning meetings Document evaluations, treatments, and progress accurately and timely Support discharge planning and caregiver education Participate in staff education and in-service training as appropriate CORE COMPETENCIES
