The Wraparound Care Coordinator facilitates team processes to help young people with behavioral health issues live in stable settings. Responsibilities include conducting assessments, developing individualized care plans, and collaborating with community partners and families.
Candidates must have a bachelor's degree in social work, psychology, or a related field, or equivalent experience. A valid driver's license and the ability to travel to client sites are required.

FamilyWise is hiring a Wraparound Care Coordinator to facilitate a team process to give families voice and choice, with the goal of helping young people with behavioral health issues to live in a stable family or community setting.
