Occupational Therapist (OT) – Home Health | Per Diem Bella Terra Home Health and Hospice is committed to providing comfort, compassion, and exceptional care to patients in their homes or facilities.

We are growing and seeking an experienced, compassionate Occupational Therapist (OT) to join our team on a per diem basis in the Inland Empire area!

Key Responsibilities Implement standards of care for occupational therapy services in accordance with the home health plan of care.
Provide patient, caregiver, and family education and counseling.
Communicate effectively with physicians and healthcare team members.
Evaluate patient’s functional status (muscle function, endurance, coordination, communication skills, self-care ability, etc.).
Assess home environment for safety and recommend adaptive equipment.
Develop individualized treatment programs and set functional goals.
Train patients in daily living activities, prosthetic/orthotic device use, and adaptive equipment.
Fabricate splints as needed.
