The Care Coordinator will assess patient needs to develop individualized care plans and facilitate communication among multidisciplinary teams. They are responsible for coordinating appointments, monitoring patient progress, and addressing barriers to care to ensure seamless service delivery.
Candidates must hold a Medical Assistant certification and possess at least 2 years of experience in care coordination or case management. Proficiency in electronic health records and strong interpersonal skills are required to effectively support a complex patient population.

Act as a liaison to facilitate communication among multidisciplinary teams, ensuring that care delivery is seamless and efficient. Identify barriers to care and coordinate necessary resources.
