The Transitional Care Coordinator provides interventions and discharge planning services to patients and families while managing community and governmental resources. They are responsible for addressing psychosocial needs, end-of-life issues, and performing duties as assigned by the Transitional Care Lead.
The role requires knowledge of disease, treatment, and psychosocial implications across various stages of development. Strong communication skills are essential for coordinating care and interacting with patients, families, and the care team.
The Transitional Care Coordinator II is knowledgeable about disease, treatment, available community and governmental resources, psychosocial implications for individuals and families as it pertains to their age and stage of development; provides appropriate interventions and discharge planning services to the patients and families; and is pragmatic in approaching end of life and ethical issues. Communication is key to the role and various modes of communication are utilized. Performs all other duties as assigned by Transitional Care Lead and Case Management Director.
