The provider will deliver primary care services to residents in long-term care facilities, including performing physical exams and diagnosing health conditions. They will also develop individualized care plans, prescribe medications, and collaborate with interdisciplinary teams to ensure high-quality patient outcomes.
Candidates must hold an active and unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician in New York. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.
