The LPN will monitor chronic medical conditions, administer routine medications, and perform clinical tasks such as EKG and drug screenings. They are also responsible for coordinating medical appointments and documenting patient responses to treatment.
Candidates must be a graduate of an accredited nursing program and hold a current NYS Licensed Practical Nurse license. A minimum of three years of related experience, specifically in providing care for chemically addicted patients, is required.

You will have responsibilities including but not limited to: Provide direct nursing care to clients under the supervision and direction of a Registered Nurse (RN), Nurse Practitioner (NP), or Physician. Maintain patient confidentiality and conduct in accordance with Phoenix House medication policies and procedures. Supervise unlicensed assistive persona; (e.g. CNA, Medical assistant, medical technician). Conduct routine nursing procedures such as EKGs, wound care, specimen collection & more. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies. Perform and document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to supervising clinicians. Observe and promptly report changes in patient behavior, appearance, or health status to the RN or provider. Provide health education and reinforce treatment plans, medication adherence, and self-care strategies as directed by supervising clinicians.
