Manage a personal patient panel while collaborating with an integrated care team of physicians, nurses, and behavioral health providers. Provide high-quality, patient-centered primary medical care to a diverse community population.
Must be a licensed PA-C or ARNP with a graduate degree. Candidates must be comfortable using Epic electronic health records and committed to community health and health equity.
