Obtain and summarize comprehensive medical/medication histories for safe and effective care

Reconcile patient medications during ambulatory care clinic visits
Perform physical assessments and triage patients as needed
Document encounters and interventions in the patient electronic medical record
Design, implement, assess, monitor, and document therapeutic plans
Initiate, modify, renew, refill, or discontinue medication therapy
Order labs and tests to determine efficacy and toxicity of medication therapy
Document/report adverse drug events (ADEs) and medication allergies
Ensure compliance with governing regulatory authorities
Review patient profiles for appropriate drug selection and dosage
Provide drug information/education to patients, families, and providers
Assess medication adherence during medication counseling
Set priorities and work independently and cooperatively within and outside the department
Present information clearly and concisely
Serve as preceptor to students and pharmacy residents
Perform other duties as assigned
Minimum Qualifications
Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
Completion of ASHP accredited PGY-1 Pharmacy Residency
Valid license from the Board of Pharmacy of the State of Texas
Certification in Basic Life Support / First Aid / CPR
Desirable: Board-Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP)
2+ years of community pharmacy experience
2+ years of medication adherence counseling experience
Experience in HIV/AIDs, Geriatrics, and/or Pediatrics
Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics
Certification in immunization delivery or smoking cessation
Computer experience Windows, Microsoft Word & Excel
Must be able to manage multiple deadlines and prioritize
Excellent communication skills and customer service skills
This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
