The Physical Therapist is responsible for assessing patient functional status, mobility, and medical conditions to develop effective treatment plans. They perform interventions to improve patient well-being and continuously evaluate progress throughout the course of care.
Candidates must hold a Bachelor's degree in a health-related field and be a graduate of a CAPTE-accredited program. A valid Physical Therapist license, CPR certification, and a valid driver's license with reliable transportation are required upon hire.

$5,000 Sign-On Bonus The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.
