The Social Worker provides psychosocial services to terminally ill patients and their families, including assessments and supportive counseling. They also facilitate community resource referrals and participate in interdisciplinary team meetings to ensure continuity of care.
Candidates must have a Bachelor's degree in Social Work with at least 2 years of experience in a healthcare or social services setting. Licensure as an LCSW is required, and the ability to work in an interdisciplinary environment is essential.

Education and Experience Bachelor’s degree in Social Work, Master’s degree preferred. Master’s degree from school accredited by the Council on Social Work Education required in states as specified. Licensure and/or certification as required by the State. Minimum of 2+ years social service experience in a health care setting or social services agency. Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs, and eligibility requirement, etc…). Strong knowledge of local community resources as it applies to the geographic area. Compliance with accepted professional standards and practices. Understands Hospice philosophy, principles of death/dying. Ability to work in an interdisciplinary setting. Demonstrates excellent observation, judgement, and communication skills. Anility to provide good written documentation within a timely manner. Self-directed with the ability to work with little supervision.
