Develop and monitor individual treatment and support plans while coordinating the person-centered planning process. Provide counseling, education, and guidance to empower individuals and assist them in accessing community services and natural supports.
Candidates must possess a Bachelor’s degree in a human services field as per Medicaid Provider Manual Guidelines. A valid Michigan driver's license is also required for this community-based role.
