The Case Assistant provides administrative support to the Community Transition Liaison, including accompanying them on nursing facility visits. They also assist consumers with public benefit and housing applications while maintaining accurate case documentation and data entry.
Candidates must have a high school diploma or equivalent, with an associate degree preferred. At least one year of relevant experience and proficiency in Microsoft Office Suite are required.
