The Care Coordinator will perform clinical screening and triage for individuals while coordinating intake, discharge, and treatment planning. They will also act as a liaison between the agency and community support systems to ensure clients receive necessary services.
A bachelor's degree in a human service-related field is required for this position. Candidates should have at least two years of related experience and possess a valid driver's license.

Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
