The Primary Care Nurse Practitioner/Physician Assistant will provide primary care services to residents in skilled nursing and long-term care facilities. Responsibilities include performing physical exams, diagnosing health conditions, and collaborating with facility staff.
Candidates must have an active and unrestricted Nurse Practitioner or Physician Assistant license in New York and a minimum of 6 months of experience in long-term care. Strong communication and clinical skills, along with a passion for elder care, are essential.
